Payment / Insurance / Cancellation Terms & Conditions
SERVICE FEES / PAYMENT RESPONSIBILITY
It is the policy of ARISE MD that payment is due at the time of service unless other financial arrangements are made and approved in advance (corporate clients and concierge members). We require all patients to make their payment prior to OR at the end of each visit. You are 100 percent responsible for all charges incurred. We do not offer payment plans or invoices for services unless agreed upon in advance. If you become aware that you are unable to pay for your services, please cancel or reschedule your appointment more than 24 hours in advance to avoid a Late Cancellation fee. Accepted forms of payment include credit card, debit card, and HSA cards.
INSURANCE COVERAGE FOR CONSULTATIONS
ARISE MD does not accept insurance or Medicare for consultations or procedures. We do not file insurance claims or handle claim resolution. We will provide a detailed receipt (specifically request a “superbill”) of services performed that you can submit to your insurance carrier if you believe you have coverage. We highly recommend you contact your insurance carrier and check into your coverage for out-of-network physicians and out-of-network specialty clinics PRIOR to scheduling your appointment.
INSURANCE COVERAGE FOR LABS/TESTING
Through our relationship with Northwestern Medical Center, some of our patients receive coverage for the routine labs/testing fees (these fees are separate from the consultation and procedures service fees). Please bring your insurance card with you to each in-office appointment in the event you require labs/testing.
When you book your appointment, you are holding a space on our calendar that is no longer available to our other patients. In order to be respectful of your fellow patients, please call our office at 414-386-2600 or via email at firstname.lastname@example.org as soon as you know you will not be able to make your appointment.
If cancellation is necessary, we require that you call or email at least 24 hours in advance. Appointments are in high demand, and your advanced notice will allow another patient access to that appointment time.
HOW TO CANCEL YOUR APPOINTMENT
If you need to cancel your appointment, please call us at 414-386-2600 between the hours of 9:00AM CST and 5:00PM CST. If necessary, you may leave a detailed voicemail message. We will return your call as soon as possible.
A cancellation is considered late when the appointment is cancelled less than 24 hours before the appointed time. A no-show is when a patient misses an appointment without cancelling. In either case, we will charge the patient a $150 missed appointment fee.
For New Patient appointments or first-time appointments, a no show or late cancellation will result in a full charge of the new patient fee.
